Before your appointment, you may receive an SMS with a link to complete this multiple-choice online questionnaire for your doctor. It is safe to click this link, but if you are unsure, please speak with our Reception team.

After Hours

If you require medical assistance outside of opening hours, please call our practice or the 13SICK National Home Doctor on 13 74 25 to be advised of after hours care options. Alternatively, you can present to the Mt Barker District Soldiers Memorial Hospital Emergency Department at 87 Wellington Road, Mount Barker (phone: 8393 1777).
